In the game there are a total of 15 hideouts of bands to find and complete to unlock this trophy / achievement. As for all other activities, even in this case you just need to find and unlock all the antennas to view the location of the hideouts on the map, marked with the fist icon (follow the ctOS antenna guide for more help).

Arrived in the area you will have to locate the target and take it out: you can act with a hard fist and exterminate all the enemies in the vicinity or take a stealth approach and hit your target directly.

Apparently several users are finding it difficult to find the last hideouts, but it seems that the only solution is to keep playing and go on with the main quest (remember as said at the beginning to unlock the ctOS antennas.)

I also propose this video that shows the location of all the lairs, but I still recommend that you continue playing and complete the lairs as you come across them throughout the game.
