This achievement can be pretty tough as you will need to get S rank in each episode playing on the hard level.
To make things easier for you, start taking the Mortal bar and then complete the game on the easy level to get the Obliterator bar. With this bar you will have more chances to get the S rating. To succeed, worry mainly about getting a lot of battle points, if you die too many times it is better to restart.
Make sure you complete them all at the required level, even the secret episode.
